With the introduction of Banelings on Legend of the Seeker this season, it is inevitable that one of the main characters would have to personally face the decision of whether to make a deal with the Keeper in order to escape the Underworld. Richard already turned Darken Rahl down when he found himself trapped in the Underworld, but Cara does not choose the same way when it is her turn to make a decision.
When Cara is given the opportunity to return to the land of the living, she characteristically makes whatever choice will allow her to continue to help and protect Richard. The majority of "Hunger" is an examination of Cara's journey toward becoming a better person under the influence of Richard, Kahlan, and Zedd, and that part of the story is compelling.
While making a deal with Darken Rahl does not seem like it would be anything that someone who allies themselves with Richard should be doing, it probably makes a lot of sense from Cara's perspective, at least at the beginning of the episode. She is so focused on being there to support Richard that it usually blinds her to any other concerns. It might have been better to see her hesitate a little more before jumping at Rahl's offer, but it also feels in character for her not to think too much about it. In Cara's world, there is an abundance of people who need killing, so she assumes she won't have too much of a problem finding candidates. It is also pretty funny to see how Cara is not freaked out in the slightest at finding herself in the Underworld.
